Two very different things hide behind the word fireplace. A masonry fireplace is built on site from brick, block or stone, with a firebox lined in firebrick. A factory-built fireplace is a listed metal unit, tested to UL 127, that arrives as a system and has to be kept to its listing, as section R1004.1 of the Florida residential code requires.

A lintel, usually a steel angle, holds up the brick above a fireplace opening. Code section R1001.7 calls for a noncombustible lintel with at least 4 inches of bearing on each end.
Steel that rusts grows in size, and that growth can crack the joints above it, push brick out of line or let the angle sag. Brick industry guidance (BIA Technical Note 19) recommends leaving room at the lintel ends so the steel can expand with heat. The grate and what it protects
A grate lifts the fire so air can reach it and keeps burning wood off the firebox floor. Factory-built manuals often specify the grate to use and warn against burning directly on the panels.
